- ワクチン3回目接種、またはPCR等検査の陰性結果の提示ができること。
※市販の検査キット(セルフチェック)は認められません
※PCR・抗原定量検査：体験日前3日以内
※抗原定性検査：体験日の前日または当日 - PCR等検査結果は、以下の項目が明記されている検査を利用すること。
a 受検者氏名 / b 検査結果 / c 検査方法 / d 検査所名 / e 検体採取日 / f 検査管理者氏名 / g 有効期限 - 氏名・生年月日が記載されている身分証明書の提示ができること。
- 12歳未満のお子さまのワクチン等接種証明書類に関しては、他の同行者全員がワクチン3回目接種証明またはPCR陰性証明類および身分証明書を提示すれば提示不要です。
※体験日当日、上記条件を満たせなかった場合は、割引適用無しの通常料金で差額をお支払いいただくか、所定の取消料をお支払いいただきキャンセルいただく形となります。
Campaign Terms and Conditions
１．Applicants must be able to provide proof of your 3 doses of a novel coronavirus (COVID-19) vaccination or a valid certificate of COVID-19 negative inspection result such as PCR tests.
*Self-check COVID-19 test kits commercially available on the market are not acceptable.
*PCR/Quantitative antigen test: within 3 days prior to the day of the activity programs.
*Qualitative antigen test: within 3 days prior to or on the same day as the activity day.
２．Conditions to be included in the inspection certificate.
a. Name / b. Result / c. Testing method for COVID-19 / d. Name of medical institution / e. Test result date / f. Signature by doctor / g. The term of validity
*Telephone communication of test results by some inspection institutes is invalid.
３．Applicants must be able to present their ID card that confirms their name, date of birth, and that they reside in Japan.
４．Children under 12 years of age do not need to present proof of vaccination as long as all other accompanying persons present proof of vaccination (3rd dose or PCR negative) and identification.
*Regarding the vaccination certificate, a photo taken with a smartphone or other device may be presented if the applicant’s name, date of birth, three vaccinations, and vaccination record can be verified as being that of the applicant.
*If you fail to meet the above conditions on the day of your activity program, you will be asked to pay the difference in the regular fee with no discount applied, or pay the prescribed cancellation fee and cancel your reservation.
